Forms needed: Annual Club Registration and Club Administrator Roster. Clubs are required to register their club annually on July 1st. Annual Club Registration form, Club Administrator Roster, $25.00 check, background disclosures on each board member, copy of current club by-laws, and a copy of the club's 501c3 are needed. A checklist is provided to the club registrars in their Registrar's Manual.

For Fall competitive U14 teams only, any U15 players still in the 8th grade are eligible to play on a U14 team when the Eighth Grade Certification/Waiver form, properly completed, is submitted to Oregon Youth Soccer Association with that player's team registration. These players cannot play in the Presidents or State Cup with the U14 team.

If you are a coach or other club administrator who needs a printed member pass for the new seasonal year, contact your club registrar for the URL to your club's registration page and submit your disclosure as part of registering for your team role. The Registration Change form is used to add, delete, or transfer a member. The club registrar must sign this form.

The team registration form is available in Microsoft Excel format for teams to use for travel papers. Club Registrars may also use this form to have Oregon Youth Soccer Association enter their registration. Data entry fee will be charged.

Out of state players need to register in their own state prior t
gistering in Oregon. This applies to players for Recreational and Competitive teams. The paper work could take 2 to 3 weeks to process. A player cannot be rostered in Oregon until we receive the release agreement from their residence state, nor should the player be training with the Oregon team until the release is complete.

This form is available to anyone who needs a medical release for club or tournament use.
